Porcelain versus composite inlays/onlays: effects of mechanical loads on stress distribution, adhesion, and crown flexure.

چکیده
This study used 2-D finite element modeling to simulate cuspal flexure and stresses at the surface and tooth-restoration interface of a restored maxillary molar using three restorative materials; the influence of four inlay/onlay preparation configurations on stress distribution within the complex was also investigated. A buccolingual cross-section of an intact molar was digitized and used to create 2-D models restored with different restorative materials (feldspathic porcelain, high- and low-elastic modulus composites) and tooth preparations (small and large inlays, small and large onlays). Two simulated 25-N oblique loads were applied to the cusps. The tangential stress for each finite element node located at the tooth surface, interfacial stress, and relative cuspal flexure were analyzed. All materials and tooth preparations exhibited similar surface tangential stress patterns, with a definite compressive area at the external cusp ridges, a tensile zone at the occlusal surface, and compression stress peaks at the CEJ. The low-elastic modulus composite showed reduced tensile stresses at its surface but increased tension at the dentin-adhesive interface when compared to ceramics. All types of onlays demonstrated a majority of compressive interfacial stresses, while inlays showed a majority of tensile stresses. The interfacial tension at the dentin level increased with the flexibility of the restorative material. Only the large ceramic onlay displayed almost pure compression at the interface. Composite-restored teeth exhibited increased crown flexure, while porcelain-restored teeth showed increased crown stiffness. Porcelain inlays/onlays featured more detrimental stresses at the occlusal surface but better potential protection against debonding at the dentin-restoration interface compared to composite inlays/onlays. Ceramic onlays/overlays seem to represent an effective answer to restore severely damaged posterior teeth.

Cumulative effects of successive restorative procedures on anterior crown flexure: intact versus veneered incisors.
OBJECTIVE When successive restorative procedures (e.g., porcelain veneers, interdental resin composite restorations, and endodontic treatment) are carried out on the same tooth, significant effects on crown flexure can be expected. METHOD AND MATERIALS Dentin-bonded porcelain veneers (experimental group) were assessed in vitro using functional and cyclic thermal loads. They were compared to n...
متن کاملCaptekTM - A new capillary casting technology for ceramometal restorations
CaptekTM (capillary technology) is an advanced metallurgic system that was developed to combine optimal natural esthetics of dental porcelain with the strength of ceramometal , to produce accurate restorations on all types of tooth preparation and provide a biocompatible environment for the oral tissues in simple and affordable procedures. CaptekTM restorations include inlays, onlays, crowns, a...
متن کاملDirect composite resin onlays: rationale and clinical application.
Ceramic full crowns can restore lost tooth structure, strength, and esthetics but remove about twice as much tooth as an onlay design. Onlay restorations can maintain facial enamel for long-term esthetics, preserve the dentogingival junction for optimum periodontal health, and use supragingival margins for predictable adhesive dentistry. Metallic onlays can create fracture strength 3 times that...
متن کاملCeramic Inlays versus Composite Inlays
Acid etching and filled acrylic materials have been generating great interest in the past few years because of increased public demand for esthetic restorations and alternatives to amalgam. Composite resin and ceramic inlays, which use adhesive bonding, were introduced to make possible toothcolored restorations. The physical properties of composite resin restorations, and their poor wear resist...
متن کاملCritical Review and Evaluation of Composite/Ceramic Onlaysversus Crowns
The purpose of this comprehensive review was to evaluate and identify the performance of composite/ceramic onlays versus crowns. The strength of the evidence is based on published trials, systematic reviews and observational studies. Methods: The dental literature was reviewed for controlled clinical studies and retrospective cross-sectional studies from 1966 to 2013. Longevity of onlay or crow...
